hmmmm I wouldn't normally be concerned except for the kitten throwing up a worm. I've never seen one do that, and yes, he might have thrown up the medicine, too.
In my experience, it has only ever taken a couple of days for the kitten to pass the worms after getting medicine. Then if it was a bad case, the vet might want to check the kitty again at 10 days or might just wait until their next set of shots.
I wouldn't wait to take him back to the vet if he is still dehydrated and weak though! Call first thing Monday morning and let them know the kitty is still ill.
In the meantime, it is important to get the kitten hydrated!! Maybe you can get him to eat some canned food or get a children's medicine dropper and drop water into the kitten's mouth a little at a time. If he continues to be dehydrated, you might take him to the emergency vet - dehydration can take a kitten quickly!
Also, I would probably separate the 2 kittens until I knew this one was okay.

If it is a longhaired cat, that might be hairballs. One of my girls used to feel all funky when she got them.
It could also be allergies or even asthma since it happened before.
Whenever mine cough, I worm them and give them a dose of hairball remedy, and the coughing normally stops. If it didn't, I'd take them in to get them checked for allergies or asthma.
Kitties can also get colds. That would normally be accompanied by runny eyes and nose, though, and those should always get checked out by a vet because upper repiratory infections are especially tough on cats.
Hope it clears up! If it didn't within a couple of days, I'd check in with the vet.

Hello! Sometimes cats do slow down a little bit as they get older and mature. If you try to get him to play, CAN he? Does he have any trouble holding his head up, or does he go limp when you pick him up?
That he's eating is a good sign. He could have some worms - have you had any fleas with him? Tape worm can especially make a kitty tired.
Also, has he gotten any worse over the 3 days, or has it been about the same?
